Background : Patients with primary adrenal insufficiency (PAI) describe significant impairments in quality of life. Current replacement regimes are non-physiological. Animal studies indicate that pulsatile presentation of endogenous glucocorticoids is critical for normal transcriptional and behavioural responses.
